SONG HOUSE
CAMP INTAKE FORM
One-Day Boot Camp  |  April 2026

Welcome to Song House.

We are so glad you are here. The fact that you said yes to this day already tells us something important about who you are. You are not the kind of artist who sits on the sideline and waits for permission. You showed up. That matters more than you know.

Now here is what you need to know: this is not a typical workshop. This camp is part development, part audition. We are going to push you creatively, give you honest feedback, and pay close attention to how you handle the room. How you respond to a challenge matters just as much as how you sound. We call it “We Are Watching” because we genuinely are. Not to judge you, but to find out what you are made of.

The answers you give in this form help us prepare for YOU specifically. So please do not perform on this form. Do not give us the version of yourself you think we want to see. Just be honest. The more real you are here, the better we can serve you tomorrow.

One more thing: we are doing pizza for lunch. If you cannot do pizza, please bring your own meal.

BEFORE YOU FILL THIS OUT

We are excited to have Adam Watts as a guest speaker and communicator at camp. Adam is a producer, songwriter, artist, filmmaker, educator, and author with over 3 billion streams worldwide, 50 million albums sold, and eight consecutive years in the Billboard Top 200. He has written and produced for artists like Demi Lovato, Miley Cyrus, the Jonas Brothers, Kelly Clarkson, Colton Dixon, Jeremy Camp, and Switchfoot. His work spans Disney, Sony, Universal Republic, Capitol Records, Chronicles of Narnia, and the Netflix film “A Week Away.” He has won 3 ASCAP Awards and 4 Dove Awards. He is also an adjunct professor at Biola University and the author of “The Human of Being Art.”

Adam created an Artist Type assessment that identifies your creative wiring across 10 different artist types. He would love for you to take this before camp so we can get a better feel for where you are and hit the ground running instead of spending time figuring out who you are in the room. The more we know going in, the more we can get straight to the point and make every minute count.

Use the promo code SONGHOUSE at checkout. You do not need to purchase the $9.99 version; this is a gift from Song House. Take it ideally before tomorrow.

Section 1: The Basics
Where are you based right now?
Just in case. We take care of our people.
Social media, friend referral, event, etc.
Section 2: Your Creative Lane
This section helps us understand your sound so we can build custom tracks and pair you with the right co-writers on camp day.
Pop, country, R&B, folk, hip hop, worship, rock, singer-songwriter, etc.
Maybe you write country but love R&B melodies. Tell us where your sound bleeds.
Not just who you listen to. Who shaped how you write, sing, or think about music?
Guitar, piano, none, a little of everything, etc.
Raspy, smooth, belty, conversational, airy, powerful, raw, etc.
If you know it, great. If not, no worries. Tyler doesn't even know the best key for his voice. Just curious.
Uptempo, mid-tempo, slow burn, or all over the place.
Anthemic, melancholy, gritty, hopeful, playful, dark, cinematic, confessional, etc.
Spotify, YouTube, SoundCloud, Instagram, TikTok. This is optional but helps us know your lane.
Section 3: Where You Are Right Now
This is the most important part of this form. We are not looking for the polished version of your story. We want the real one. The more honest you are here, the better we can coach you.
Not the dream version. The honest version. What does your real creative life look like if it all works out?
Financial, emotional, relational, creative, logistical. Whatever it is.
Something you have carried or are still carrying that is not true but has shaped how you move.
You do not have to share details. A yes or no is enough. But if you want to say more, this is a safe place to do it.
Do you crave it? Does it shut you down? Have you ever received feedback that changed how you approach your craft?
The one thing you want to walk away with.
Section 4: Your Experience
Optional. Link your primary platform.
Yes, no, or it is complicated.
Section 5: Your Socials
Drop your handles so we can check out what you are doing online.
YouTube, Spotify, X, SoundCloud, etc. Drop links or handles.
Section 6: Your Artist Type
If you have not taken the Artist Type assessment yet, please do so now at myartisttype.com using the promo code SONGHOUSE. It is free with the code; this is a gift from Song House. Take it ideally before tomorrow. Adam will reference your results during his session, so having this done ahead of time lets us skip the intro and go deeper faster.
Visionary, Consummate, Analyzer, Tech, Entertainer, Maverick, Dreamer, Feeler, Tortured, or Solo.
Section 7: Video Check-In (Optional)

This part is completely optional. You do not have to do this, and there is zero pressure. But if you are interested in telling your story and potentially being featured on the Song House website, in before-and-after content, or as part of a testimonial following your camp journey, we would love to see a short video from you before you arrive.

A few ground rules on how we use content at Song House: we are here to encourage. Period. We are never going to make you look bad. We are never going to use your footage in a way that embarrasses you or paints you in a negative light. If you are featured in anything, it is because we want to celebrate your growth and your story.

“Record a 60 to 90 second video. One take. No edits. Tell us how you are feeling right now. Are you excited? Nervous? Scared? Hopeful? What do you want out of this experience? Just talk to us like we are already in the room together.”
Upload to Google Drive, YouTube (unlisted), Dropbox, or wherever is easiest and share the link.
Section 8: Anything Else
Accessibility needs, emotional triggers, things that make you feel safe, things that shut you down, or just anything on your mind. This is your space.
